Eligibility Requirements
Eligibility to enroll for the Voluntary Accidental Death and Dismemberment Insurance Policy is as follows:
- You must be a current Independent Business Owner (IBO) under the age of 70 and member of the Independent Business Owners Benefits Association.
- You must be a citizen or legally able to work in the United States.
- You must not be confined in a hospital.
- If your spouse is a current IBO, you are both eligible for the IBO Only Plan coverage, however, he/she may apply by completing a separate enrollment form.
Note: If you and your spouse are current IBOs, both are eligible for coverage.
Limitations and Exclusions
This policy does not cover suicide or any attempted suicide while sane or insane, loss caused by an act of declared or undeclared war; injuries received while participating in training exercises or maneuvers while a member of an armed service; injuries received while traveling by air (except as specifically provided), injuries received while under the influence of any controlled substance, unless administered on the advice of a physician, or injuries received while the insured person is intoxicated. Refer to your certificate of insurance for the details, limitations, and exclusions of the Accidental Dealth and Dismemberment benefits.
Age Reductions
Principal Sum Benefits for any Insured age 70 and over shall be payable in accordance with the following schedule:

Coverage terminates |
Voluntary Accidental Death & Dismemberment Termination
Coverage for you and your dependents ends on the earliest date:
- The policy has terminated.
- You are no longer eligible for insurance under the policy.
- The date any premium is due and unpaid, subject to any grace period.
- Coverage for each dependent will terminate the last day of the billing period following the date the person ceases to be an eligible dependent.
