Open Enrollment for New Independent Business Owners
USI Affinity is pleased to offer open enrollment of guaranteed issue amounts for term life insurance for eligible, U.S. Independent Business Owners (IBOs) who are not currently insured under the IBOBA group term life (162250-1-G) policy. Take advantage of this offer by enrolling within 60 days of becoming an IBO. During open enrollment, guaranteed issue means IBOs may enroll for certain amounts of coverage without answering health questions.
IBOs may enroll for a guaranteed issue amount of $50,000* in annual term life coverage without answering health questions. You may also include up to $10,000 of coverage for your dependent children who are dependent upon you for support and under the age of 26. If your spouse is also an IBO, they may also enroll as a member. If your spouse is not an IBO, you may enroll your spouse as a dependent for a guaranteed issue amount up to the same amount of coverage purchased by the IBO.
Open Enrollment Eligibility: If you are a current IBO (not currently insured under the IBOBA group term life 162250-1-G policy), you may enroll for the guaranteed issue amount. You must be under the age of 70 and a member of the Independent Business Owners Benefits Association, a citizen or permanent resident of the U.S., able to perform normal activities, and not be confined at home, in a hospital, or in any other care facility. Some non-preferred industries will limit the total amount of voluntary term life insurance available under this IBO group program to $25,000*.
Eligibility Requirements
Eligibility to enroll for the Voluntary Group Term Life Insurance Policy with optional Accidental Death & Dismemberment Insurance Rider is as follows:
- You must be a current Independent Business Owner (IBO) under the age of 70 and member of the Independent Business Owners Benefits Association.
- You must be a citizen or able to legally work in the United States.
- You and your eligible Dependents must be able to perform normal activities and not be confined (at home, in a hospital, or receiving social security benefits).
- If your spouse is a current IBO, he/she may apply by completing a separate application.
- Spouses who are not current IBO’s may be enrolled providing coverage has been approved and issued on the current IBO.
- Dependent children, 14 days old up to age 26 (and dependent upon you for support), are eligible providing coverage has been approved and issued on the current IBO.
Note: If you and your spouse are current IBOs, both are eligible for coverage as a member.
Limitations and Exclusions
Voluntary Term Life
Life insurance benefits will not be paid if the insured’s death is the result of suicide within two years from the date of issues. If this occurs, the sum of the premiums paid will be returned to the beneficiary.
If you or your spouse is engaged in the following non-preferred industries, you can only enroll for $25,000 of Voluntary Group Term Life coverage.
*List of Non-Preferred Industries
- Husbandry: commercial fishing; hunting and trapping
- Mining: coal mining; metal mining; underground mining; chemical and fertilizer mining; oil and gas field extraction
- Climbing: steeple-jacking; antenna installation
- Destruction: ammunition and explosives; wrecking and demolition; asbestos
- Vehicle: taxicab and bus drivers; water transportation; commercial and non-commercial pilots; automobile and motorcycle repair; professional sport racing; amusement parks; flight instructors
- Waste: refuse and sanitation systems (sewage); scrap and waste materials
- Justice: bail bondsman; justice, public order and safety (police, fire and ambulance); military personnel; detective and armored car; security guards and any occupation which requires carrying a weapon.
- Other: logging and saw mills; nuclear energy
Age Reductions
Coverage amount(s) will reduce according to the following schedule:

Accidental Death & Dismemberment (AD&D)
IBOs may add an optional AD&D benefit which offers protection 24 hours a day against a covered accident in the course of business, or pleasure, whether on or off the job, or in or away from home. Loss means with regard to a covered accident which causes:
- Loss of life,
- Loss of limb,
- Loss of vision, hearing or speech
- Paralysis
This provision does not cover suicide or any attempted suicide while sane or insane; loss caused by an act of declared or undeclared war; injuries received while participating in training exercises or maneuvers while a member of an armed service; injuries received while traveling by air (except as specifically provided); injuries received while under the influence of any controlled substance, unless administered on the advice of a physician; or injuries received while the insured person is intoxicated. Refer to your certificate of insurance for the details, limitations, and exclusions of the AD&D benefits.
Voluntary Term Life & Accidental Death & Dismemberment Termination
Coverage for You and your dependents ends on the earliest of:
- The date You reach the Attained Age of 70; or
- The date You begin active duty in the Armed Forces, National Guard or Reserves of any state or country or
- The date the Group Policy Ends; or
- End of the period for which the last premium has been paid by You

IBOBA: When you register or renew as an IBO, you also become a member of the Independent Business Owners Benefits Association (IBOBA). The IBOBA is a non-profit association which promotes the interests of its members at the local, state and federal government levels, to use the power of group buying to make available to the members benefits and services such as travel programs, including automobile rental and hotel discounts, insurance, non-insurance healthcare discounts, and to provide educational information and forums. This insurance is just one benefit arranged through the IBOBA.
