Open Enrollment for New Independent Business Owners
USI Affinity is pleased to offer open enrollment of guaranteed issue amounts for long-term disability for eligible U.S. Independent Business Owners (IBOs) currently not insured under the IBOBA group long-term disability (162250-1-G) policy. During open enrollment, guaranteed issue means IBOs may enroll for certain amount of coverage without answering health questions. Take advantage of this offer by enrolling within 60 days of becoming an IBO.
IBOs may enroll for up to 60 percent of their before tax monthly earnings up to the guaranteed issue amount of $4,000* without answering health questions or undergoing a medical exam. Should you wish to apply for an amount exceeding $4,000 (and up to $10,000*) or if you miss your enrollment opportunity, you will be required to furnish evidence of insurability. If your spouse is also a current IBO meeting the IBO Eligibility, he/she may also enroll by completing a separate enrollment form.
IBO Eligibility: If you are a current IBO (not currently insured under the IBOBA Group Long-Term Disability 162250-1-G policy), you may enroll for the guaranteed issue amount. You must be under the age of 60 and a member of the Independent Business Owners Benefits Association, working a minimum of 30 hours per week, a citizen or legally able to work in the U.S., able to perform normal activities, and not be confined at home, in a hospital, or in any other care facility. Some occupations/industries will limit the total amount of long-term disability insurance available under the IBO group program.

*IBOs in Non-Preferred Industries are limited to Maximum Monthly Benefit of $2,000 and a Guaranteed Issue Amount of $500
Elimination Period
This is the amount of time before benefits begin, if you become disabled. This plan has a 90-day elimination period.
Guaranteed Issue Monthly Benefit Amount
You may elect a guaranteed issue monthly benefit amount from $500 to $4,000, in increments of $100. The maximum monthly benefit is $4,000, not to exceed 60 percent of your before-tax monthly earnings.
*For Non-Preferred Industries you may elect a guaranteed issue monthly benefit up to $500 not to exceed 60 percent of your monthly income.
Should you wish to apply for a higher monthly benefit than your guaranteed issue amount, you will be required to furnish evidence of insurability.
Maximum Benefit Period
If you become disabled, benefits are payable up to the Maximum Benefit Period of two years, subject to any limitations described in the Certificate.
Definition of Disability
Due to a Sickness, or as a direct result of accidental injury:
You are receiving Appropriate Care and Treatment and complying with the requirements of such treatment, and is unable to earn more than 80% of your predisability earnings at your Own Occupation for any employer in their Local economy; and is unable to perform one or more of the material duties of your Own Occupation.
Definition of Monthly Earnings
Monthly Earnings is the average net monthly earnings from salary, wages, bonuses, commissions, fees or other payments received for personal services rendered or work performed in any occupation. The average is based on your net monthly earnings for the most recent 12-month or 24-month period, whichever produces the higher average. Normal and usual business expenses are to be deducted. Income taxes are not to be deducted. Monthly earnings does not include dividends, rents, royalties, annuities or other forms of unearned income. Proof of Earnings is required.

Effective Date
An eligible IBO member will become insured, subject to payment of required premiums, on the first day of the month that follows the latest of the day:
- The IBO member begins Active Eligibility.
- The IBO member submits a written request to enroll for insurance.
- Following the approval of Evidence of Insurability, if required.
Your Monthly Benefit Will be Reduced by Other Income Sources
Income you earn from other sources accounts in determining the amount of your monthly benefit. This includes:
- Disability benefits under Workers’ Compensation Law, occupational disease law or similar law.
- Disability benefits under the United States Social Security Act, the Canada Pension Plan, the Quebec Pension Plan or similar plan or act that the insured person or the insured person’s spouse and children are entitled to receive because of the insured person’s disability.
- No-fault automobile insurance plan.
- Retirement benefits under the United States Social Security Act, the Canada Pension Plan, the Quebec Pension Plan or similar plan or act that the covered person receives because of his or her retirement, unless the insured person was receiving them prior to becoming disabled.
