With our IBO vision insurance benefits program, you have the freedom to visit network or out-of-network providers. CIGNA Vision Network offers one of the largest national routine vision networks with more than 66,000+ optometrists and ophthalmologists at over 24,734 locations nationwide, including private practice and national and regional retail locations.

Please note: Frequency is 12 months for exams, 12 months for lenses and contact lenses, and 24 months for frames.
In-Network Benefits
- One vision and eye health evaluation including, but not limited to:
- Eye health examination
- Dilation
- Refraction
- Prescription for glasses
- One pair of prescription plastic or glass lenses, all ranges of prescriptions (powers and prisms), including:
- Polycarbonate lenses for children under 18 years of age
- Oversize lenses
- Rose #1 and #2 solid tints
- 20 percent savings on non-covered lens options
- Progressive lenses covered up to bifocal lens amount with 20 percent savings on the difference
- One frame of choice covered up to retail plan allowance, plus a 20 percent savings on amount that exceeds frame allowance
- One pair or a single purchase supply of contact lenses – in lieu of lenses and frame benefit, (may not receive contact lenses and frames in same benefit year). Allowance applied towards cost of supplemental contact lens professional services (including, the fitting and evaluation), and contact lens materials
- Value added savings, include:
- 20 percent savings on additional purchases of frame and/or lenses, including lens options (with a valid prescription)
- Up to 15 percent savings on the contact lens professional services (fitting and evaluation). Offered savings does not apply to contact lens materials

The above rates are valid until December 31, 2024.
*Dependent Children Eligibility: any unmarried child of yours who is less than 26 years old. Dependent children eligibility and age limits may vary by state – refer to the CIGNA insurance certificate for specific state variations. This rate applies to all eligible dependent children regardless of the number of children covered under your plan.
Quality Service
You will receive quality service from CIGNA’s network of eye care professionals. All network professionals are credentialed according to the National Committee of Quality Assurance (NCQA) standards.
Once you are enrolled in the CIGNA Vision Plan, you will be serviced by an experienced team of CIGNA Member Service professionals ready to assist you.
Eligibility for Enrollment
If you are a current IBO and a member of the Independent Business Owners Benefits Association (IBOBA) or spouse and/or dependent child, a U.S. citizen living in the United States and hold a social security number, you may enroll.
Once enrolled in our vision benefits program, you will not be required to satisfy a waiting period before using services; however, should your coverage terminate, for any reason, you (and your eligible spouse and/or dependent children) will have to satisfy a 12-month waiting period to re-enroll.
Getting Started
To get started, complete a CIGNA Vision Enrollment Form and submit, along with your initial premium to USI Affinity. Your coverage will be effective on the first day of the month following approval of your enrollment form providing initial down payment has been paid.
- How do I find a convenient network vision provider? You may visit the CIGNA website to locate a provider or call CIGNA Vision at 1-877-478-7557
- Do I need a referral to visit an eye care provider? No! You may visit any vision care provider in the network or out-of-network.
- Can family members use different eye care providers? Absolutely! Each covered family member is free to select his or her own eye care provider.
- Can I visit an out-of-network eye care provider? Yes, you may; however, you will be responsible to pay the out-of-network provider (in full) when services are rendered.
Who files the claim and do I get reimbursed? When you visit a participating network vision provider, you do not have to file a claim form and there is no waiting for reimbursements. You will receive your examination and eyewear benefit at the time services are rendered. When you visit an out-of-network vision provider, you will be required to submit a completed CIGNA Vision Claim Form along with your itemized receipt to CIGNA Vision for reimbursement.
