Extended Health and Travel Health Plan Benefits (British Columbia residents)
USI Affinity offers IBOs an extended health care program to supplement your basic provincial plan and provides coverage for many important medical expenses not paid for by your provincial health plan. You are covered for a 12-month period commencing with the first month in which your coverage is effective and pays a maximum of $10,000 for qualified medical expenses per benefit year.
The services outlined below are eligible for coverage, as long as they are usual, reasonable, customary, and medically necessary for the treatment of an illness or injury.
Preferred Hospitalization Accommodation
You will be reimbursed for hospital charges (up to 50% of the difference in cost between a standard and semi-private accommodation) when occupying an active treatment bed in a public general hospital.
Medical Services
The following medical services are also covered:
- Ambulance transportation
- Private duty nursing ($5,000 per benefit year)
- Accidental dental coverage
- Laboratory and diagnostic services
- Durable medical equipment
- Audio ($300 benefit provided every five years)
- Blood and blood products
- Prosthetic appliances
Other Medical Services
The following supplemental services (if performed by a licensed and/or registered practitioner) are included in the plan (subject to a 20% co-payment and limited to $300 per practitioner calendar year):
- Physiotherapist
- Clinical psychologist
- Speech therapist/pathologist
- Chiropractor
- Osteopath
- Podiatrist
- Chiropodist
- Naturopath
Vision Care
You will receive vision care benefits for the following services (up to $100 per person every 24 months):
- Prescription glasses or contact lenses
- Replacement parts for prescription eye glasses
- Medically necessary contact lenses
- Sunglasses prescribed by a medical practitioner for the treatment of ophthalmic diseases or conditions
Travel Health Coverage
Our comprehensive extended health and travel health plan benefit covers the hospitalization, medical services, and medical aid benefits outlined below in case of a medical emergency while traveling outside Canada. The plan covers a 60-day per trip maximum and up to $50,000 per benefit year.
Hospitalization
The following hospitalization charges are covered while traveling:
- An emergency medical condition
- Hospital accommodation
- Laboratory tests and x-rays
- Local ambulance services
Medical Services
You will be reimbursed for the following medical services while traveling:
- Inpatient and outpatient hospital charges
- Physician charges
- Private duty nursing fees
- Paramedical services
Medical Aid
You will also be reimbursed for the following services while traveling:
Prescription Drugs
- Cost of casts, braces, and crutches
- Wheelchair rental
- Other medical appliances (prescribed by a physician)
Prescription Drug Coverage
- You will be reimbursed for prescription drugs up to an annual maximum amount of $800 per person per benefit year (subject to a 20% co-payment per prescription). Payment of prescription drugs are based on the lowest priced generic equivalent (unless no generic equivalent available).
Dental Coverage
We also offer comprehensive, affordable dental coverage to help keep your mouth and your family’s mouths healthy. You may purchase dental insurance as a separate coverage or you may include it with your extended health and travel health coverage.
